You mark the L5 and place your thumb approximately 5 cm below this mark and your index finger aproximately 10 cm above this mark. Then, you ask the patient to touch their toes. Less than 5 cm increase in distance between your thumb and index finger, might suggest decreased mobility of the lower back.
