Example of inefficient referral letterThank you for seeing this pleasant, middle-aged man, who is a truck driver and makes a lot of kilometers on the road. He came to me today with low back pain, which he indicates to be really bad and it prevents him from going on these long road trips for work. As a consequence he loses income. As you can understand this is a big problem for him. I was thinking you could be of help to relieve this terribly invalidating back pain. I have examined him and did the following: Inspection-normal; palpation-normal; percussion-normal; range of motion-normal; special tests-normal. Power-normal; sensation-normal; reflexes-normal. I would like you to assess his back as well, because two people know more than one. Maybe you can find a cause for his back pain. As well, do you have some kind of therapy that helps relieve his pain? We would both be really grateful. I am looking forward hearing from you,
